The Dynamic Form Builder is a draggable design block that creates a unique instance of the form on the page where you drag it.  Within the settings you can choose to create multiple field types such as text input; check box selects, drop down selects, and radios.  You can specify which fields the form field maps to in the data base so the information you collect can be associated to either a people record or organization record. You hcan choose to have the form submit a notification email to a specified group when he form is completed/submitted.
